The address for Camp Ellie Mae is intentionally NOT posted on our website or any social media. Please do NOT post the address or mark your location on social media posts and photos. Additionally, all photos of any camper, other than your own, must be reviewed by a member of the Camp Ellie Mae leadership team prior to posting. Thank you for adhering to these important safety precautions.

 

Please read and sign below. The completion and submission of this form and signature page completes the Camp Ellie Mae registration process. You will see a link to the Camp Ellie Mae Information Packet on the confirmation page. Please review carefully. It will also be available for review on the website under the Resources page. If you have any questions, please don’t hesitate to email us.

 

Pool, trampoline, zipline, archery, tree climbing, and all other camp activities

  • Campers are prohibited from the use of the pool without the supervision of an adult volunteer. Any camper whose parent/guardian indicated the need for a life-saving device to be used by the camper while swimming will not be allowed in the pool or pool area without that device. As a reminder, the camper is responsible for bringing their own device, clearly labeled with the camper’s name. If the camper’s device malfunctions, Camp Ellie Mae will notify you (parent/guardian), and provide the camper with a similar device, upon your verbal approval, and email reply.
  • Campers are prohibited from the use of the trampoline without the supervision of an adult volunteer. All campers using the trampoline must adhere to the rules of Camp Ellie Mae. Failure to do so may result in the camper being prohibited from using this piece of equipment. The trampoline will be used by several campers at a time, in adherence to the trampoline manufacturer’s manual.
  • Campers are prohibited from the use of the zipline without proper equipment and supervision of an adult volunteer.
  • Campers are prohibited from the use of archery equipment without the supervision of an adult volunteer.
  • Campers are prohibited from participating in tree climbing without proper equipment and supervision of an adult volunteer.
  • Campers are expected to follow all rules and guidelines set by Camp Ellie Mae, ensuring the safety of all involved.
  • Parents/guardians assume responsibility for the risk of injury involved in camp activities.

 

Medication provided by parent/guardian

Sending medication(s) vitamins, supplements, gummies of any kind with the camper:

  • Drop-off: You will speak to a member of our staff, regarding all the information you provided.
  • During camp: A member of the staff will follow your detailed instructions provided in your camper's registration. If we have any questions, we will call you. Please be readily available for these purposes.
  • Pick-up: Please remember to pick up the camper’s medication as part of the pick-up process. Please do not retrieve the camper’s medication(s) without the assistance of a staff member.
  • IMPORTANT NOTE: ALL medications, vitamins, supplements, gummies of any kind MUST be given to camp staff, and must not be kept with the camper. This is to ensure the safety of all campers.

 

Medication (over-the-counter, "OTC"), provided by Camp Ellie Mae

  • Any authorized OTC medication indicated in the registration process will be administered by a member of the Camp Ellie Mae staff only. If we have any questions, we will call you. Please be readily available for these purposes.

 

Misc.

  • Campers are prohibited from bringing any electronic devices to camp. This includes, but is not limited to, cell phones, iPads, etc.
  • Campers are prohibited from bringing any food to camp due to the potential of attracting bugs, rodents, choking, allergies, etc.
  • Campers are prohibited from leaving the camp grounds for any reason without the permission and supervision of an adult staff member.
  • Campers will only be released to person(s) listed in the registration form. Be prepared to present a photo ID if requested. Campers will not be released to anyone who is not listed.

 

As parent/guardian of camper, :

  • I understand and acknowledge that I am fully responsible for any and all damage to the property, buildings, grounds, persons, and/or personal property of campers and/or staff, as well as any other liabilities, injuries, or damages caused by the camper during the course of the entirety of camp. Furthermore, I agree to indemnify, defend, and hold harmless Camp Ellie Mae and its employees, agents, volunteers, and representatives from any claims, losses, or expenses arising out of such damages, injuries, or liabilities. This includes, but is not limited to, legal fees, medical costs, and any other expenses associated with such incidents. I also understand that it is my responsibility to ensure that the camper adheres to all rules and safety regulations set forth by Camp Ellie Mae.
  • I understand and acknowledge that Camp Ellie Mae reserves the right to dismiss any camper(s) who disregard camp policies, engage in behavior that disrupts camp operations, or are found in possession of alcohol, drugs, weapons, or any other prohibited items. I further understand and agree that I am fully responsible for any and all costs associated with the immediate removal of the camper(s) from the premises, including but not limited to housing, transportation, supervision, and any additional expenses incurred due to the camper's removal. Additionally, I acknowledge that such dismissal may occur without refund, and I agree to indemnify and hold harmless Camp Ellie Mae, its employees, agents, volunteers, and representatives from any liabilities, claims, or damages arising from the dismissal or removal of the camper(s).
  • I hereby authorize Camp Ellie Mae staff, volunteers, directors, and any other individuals associated with Camp Ellie Mae, to seek and arrange emergency medical care for the camper if deemed necessary to preserve the health and well-being of the camper. I understand that this may include transportation to a medical facility or hospital and the provision of necessary treatment. I release and hold harmless Camp Ellie Mae, its staff, volunteers, directors, agents, and any other individuals associated with Camp Ellie Mae, from any liability, claims, or damages resulting from decisions made in good faith regarding emergency medical care or treatment. I further agree to assume full responsibility for any and all costs associated with the camper's transportation, medical care, and treatment, including but not limited to, hospital fees, doctor fees, and emergency services, and agree to indemnify and hold Camp Ellie Mae harmless from any financial obligations or liabilities related to such care.

 

By signing, I agree that the information provided is accurate. I have received, read, understand, and agree to instruct the camper to abide by Camp Ellie Mae rules. By signing, I release the staff, volunteers, and directors of Camp Ellie Mae; Paul Woods, Ellen Woods, owners of the property leased to Camp Ellie Mae, or anyone else associated with Camp Ellie Mae from any liability resulting from any harm and/or adverse incident that may occur to the camper while on the property or during their attendance at Camp Ellie Mae.
